Forms for the 2017-18 Band Season

Orders can also be placed online using through the band store. Please be sure to click Add Note to Seller during the check out process and Enter the Students Name.
The District Activity Fee is $100 per student or $200 per family. The form below should be used to take advantage of the AZ tax credit. Payment should be made directly to the Bookstore during the first week of school.
PUSD requires ALL students involved in a school sport receive a physical exam prior to participating in any school sporting activity (Marching Band is considered a school sport). This must be completed prior to band camp.
We need every parent to help in some way with the band. Please complete this form and turn it in to any IBCF officer. Thank you!
